Dear Sir I worked at Mangalchand Telecom Pvt. Ltd, Patia, Bhubaneswar, , My PFNo. was OR/6439/001. Sir I got a SCHEME CERTIFICATE from Kolkata PF Office , that certificate( Original Copy) was submitted throgh my company (Mangalchand Telecom Pvt. Ltd) on 23.08.05. The receipt copy with original seal from Regional PFoffice, Bhubaneswar is kept with me. I leaved the job on 28.09.07, and join at Dharampal Satyapal Ltd , Guwahati. From Guwahati factory (PF NO.AS/5789/ 10 ) I applied for Transfer of my PF dues (FPF& EPF along with Scheme certificate) to my Guwahati Account.
My question is
1) In ANNEXURE-K that you sent to guwahati PF office Is contained with my FPF & EPS both?. Most probably you did not mention my 8.33% i.e my FPS AMOUNT.
2) you did not send my SCHEME CERTIFICATE to guwahati PF OFFICE.
You are requested to acknowledge me the total picture and send me the same in my Hyderabad PF OFFICE.
BY THE BY I WANT TO SAY RIGHT NOW I AM IN SHRINATH ROTOPACK PVT. lTD, KATTEDAN , HYDERABAD(PF. NO. AP/47973/80),
The Annexure-K that send by Guwhati to Hyderabad Office there I did not see any FPF Account mentioned and no Scheme Certificae . That is why I want to confirm first from your Bhubaneswar Office. After getting your confirmation I will Make quirees to Guwahati PF Office.

There are no FPF or EPS details send in Annexure-K. Only date of joining and leaving is mentioned.
